Juvenile Card Registration

This form is to be filled out by parents or guardians of children fifteen years old or younger. Those sixteen years old or older may use this form. Once you have submitted the form, please allow our staff 24-48 hours to process the registration. You will receive an email with your child's card number so that you may access online resources and check out physical materials. If you would like a physical library card, you may pick up at the branch you specify on this form.
